About
The Forget Me Not Foundation of Northwest Tennessee is a growing nonprofit foundation established to provide assistance to the families of loved ones who have been diagnosed with Alzheimer's or a related form of dementia residing in Weakley, Obion, and Lake counties in Tennessee.
Alzheimer's disease is the 6th leading cause of death in the United States. Approximately 16.1 million Americans provide unpaid care for people with Alzheimer's or other dementias. These caregivers provided an estimated 18.4 billion hours of care valued at over $232 billion. On average, a family can spend $2400 a month for care of their loved one, which for most, is difficult to accommodate.

The lack of financial ability causes untrained family members to become caregivers of their loved ones. This change in family dynamic causes fear, guilt, frustration, embarrassment, and anger, making it difficult to cope with the situation that can admittedly become challenging.

Our foundation provides trained caregivers, medication assistance, incontinence supplies, and safety assurance for those with a confirmed diagnosis of dementia. We take on the role of the caretaker, which allows families to enjoy more time with their loved one.
Our Mission
We are dedicated to bringing Alzheimer's and dementia awareness to the forefront through methods of education, community involvement, and providing a higher level of care to those living with dementia.

In pursuit of our mission, our actions will be guided by dignity, integrity, compassion, teamwork, and collaboration.
Our Vision
We want to be the utmost resource for Alzheimer's and dementia families until the disease is eradicated.
